I don't know what is making UX crash, I can tell that it crashes when the POL viewer switches to full screen mode. From what I read, it seems that the problem may differ from one person to the other. Anyway, here's my log after it crashed:

Disable Packetsniffer?

I've also heard of some peeps still having some Gamepad problems, and if looks like you have a ps2 > usb converter, so I'm not sure whats supported and whats not.

Remove the gamepad from your computer.

Actually, before you remove it try disabling gamepad support in the pol and ffxi configs.

hehe doh! thanks :-)

Removing the support from the config would be a good start.

If that does not work then try removing it from your computer. Not sure since the update that any gamepads work anymore. If someone was able to get it working please let me know.

Thanks for the info, guys. It turns out that just disabling the controller in POL (I don't use the controller in POL anyway) fixed the problem!
