CCP Bans

Does anybody what methods CCP uses to ban ppl? Ive heard they IP ban but also that they ban by name/CC info. Anybody know someone thats been banned before and if they were able to play again?

account level, ip, and CC are all banned....i have a friend on thier list currently. his was a perma ban but was talked down to 30 days.
