I had a lore item in the vault of my house (set to sell) and I while I was out hunting I was able to loot another of the exact same item.
I even checked the broker and then my vault to make sure I wasn't going crazy. Has anyone else seen this?

No, he is refering to the server somehow ignoring some lore items if you have them in your vault. I also noticed this. I had 2 skeleton arms and 2 collectable books (the free one form the mage tower) at the same time. One was in the vault, 1 in inventory (of each). I even moved both items to my normal inventory in seperate slots, and the server did not ping me the usual (1 lore item of any type at a time message).
In fact if you'd like to have a huge amount of books in your house with no effort for a quest, go to the SQ mage tower, click on the book that gives you the free book (different book formt ime to time, can't recall exact spot), place it in your house, and keep doing it over and over.
Not the greatest bug in the world, but if peope can figure out what needs to happen so a Lore item is no longer lore witht his method, it might be a nice little exploit.

In my case it was gnoll tails. This might be cool if you have a lore dual wield item that you want two of. Could work with rings or bracers too I suppose.
Hope this helps someone out.

They mentioned when explaining the new trade system comming up that, when an item is put up for auction, it is removed from inventory and placed in a 'holding area'. There may be a similar system in play here- items for sale from vault are technicaly removed from your inventory, and placed in a seperate pool- so when it checks your inventory to make sure you don't already have a lore item, its not seeing the original item, as its no longer in your inventory. If that makes sense.

Seems to jive with my thoughts. So process to test would be to put a lore item you want two of up for sale in the vault (make the price silly so no one will buy it). Then go find/buy a second same lore item. Than you should have two with no problem. I'll test this tonight.
