Exar Kun is by far the hardest heroic in the game, the others can be figured out with little effort, where as this one, you MUST do certain things in a certain way.
GROUP MAKEUP
You'll Need
At least one jedi, preferably light, if he can't tank 2 gold bar bosses for 5 minutes, take two.
2 Medics, 3 if you think you need them.
All out damage dealers for the rest, a smuggler would help, but 3 or 4 mandos always works.
PHASE 1
One of the easier phases, before you start, make sure everyone is past the stone door, and inside the room with the Open Hand of Hate. Make sure your particle limits are set to max for the duration of this instance, as there are certain parts where you need to know what things are on the ground.
To start things off, have a jedi (LS is preferred, but not 100% necessary) hit him once, and start running in circles on the map, using saber throw when necessary and/or force enrage. Cultists will become hot throughout the first part of this fight. everyone else in the group should focus on these, if a medic has Thyroid Rupture, then have him keep that on the Open Hand. Once the last of the Cultists are down, have everyone attack the Open Hand, Ventrilo, or in-game voice chat helps a TON in this part, as you'll see a debuff that looks like the commando base of operations, when someone who can't take alot of damage in your group has it, STOP ATTACKING until the debuff expires. If you think your medics can handle it, just go all out on him, and heal/rez as needed. Also green circles will spawn, so it is recommended that you move away from those, as they can take significant chunks of health. Once he's down, proceed to the next room.
PHASE 2
Easiest Phase of the instance, kill the creatures in the hall to spawn the boss.
3 gold bars show up, The Minder of Weak Wills, Vin'rith, and Mother Luresh.
Have a jedi tank the minder, and someone else tank Vin, and everyone else on the mother. Be sure to stay grouped together, and let AOEs do most of the damage, Vin and Mother should go down at a moderate rate, random NPCs will spawn, but AOE damage makes VERY short work of them. Once Mother and Vin are down, focus on the Minder, he has a buff that can be purged, but it won't do a thing, he should drop pretty fast. Onto the next room.
PHASE 3
The rest of this instance can be a huge trouble, so its important you do things right. Make sure everyone is buffed up and ready to go. you'll want to stand on the side the Caretaker is.
Have a jedi or commando shoot him, commando's can stack DoTs but occasionally he'll hop out of his square before you can hurt him, making it EXTREMELY difficult to complete. So just be warned. After a few seconds a Tomb Guardian, Calming Influence, and Pain Bringer will spawn. Bring them down, I usually hit them CPT, calming, pain, tomb. Once they're down, wait another 10 seconds or so, and a prisoner will start talking, bring CPT down as they come, eventually "The Executioner" will become hot, when you see this completely devastate him, hit him as hard as possible before he gets to the prisoner. If he gets 3 prisoners before you kill him, you fail and have to restart. Once the Executioner is down, the prisoner will continue walking, some CPT will spawn, then the Caretaker Protector will start talking and become hot, bring it down and kill it, rinse and repeat until all protectors are down. (4 total) Once they are down start running and shooting the Caretaker of the Lost, avoiding the green gas that will spawn. He should go down pretty easily. Onto the next room!
PHASE 4
Kill all the tomb guardians scattered throughout. Buff up, this is the toughest part of the instance imo.
There are many ways to go about this, I'll use my most trusted version.
Position your group on either side of the bridge, having one medic on each side, and jedi in the middle.
One of the jedi must shoot the caretaker and bring him to the center, if you're lucky, he'll use fists rather than a sword, if he does, have the jedi keep aggro and beat him, if not, its a little more complicated.
Have the jedi tank for 10-20 seconds, with medics healing and performing rupture. (One medic must keep rupture on the CFoH) Once the jedi has aggro, have everyone start shooting, free to use specials. If the jedi goes down (he most likely will..) start running all over the place, fire will start spawning, if you catch fire, run to the entrance and wait for it to expire, then run back in. Eventually he'll go down, sorry, there's not much of a better way to do this.
PHASE 5
This is it, the final part. Get buffed and ready, the map is set up like a big square, x is signified by a jedi/tank, M is signified by a medic, and G is the rest of the group.

Have a commando with all elementals (No Heavy Weapons) shoot Unquenchable, and use the element that corresponds the Unquenchable's toolbar, Jedi will tank Lingering and Embraced, which will switch targets between the tanks, everyone else on Promised. This phase depends on the medics and tanks doing their job, the Elementalist MUST do their job, or your entire group will be completely wiped out, Elementalist also cannot use specials. Promised isn't anything special, just hits hard, the medic in point M, should stay still so everyone but the tanks get their heals. Once promised is down, everyone in the group besides tanks should focus on Unquenchable, they don't need to use correct element, but cannot use specials. Once Unquenchable is down, everyone but commandos need to switch melee and hit lingering and embraced, you'll see a buff on your toolbar which will tell you one you can actually deal damage to. Once they are down, you've pretty much got this in the bag.
An Exar Kun Wordbearer will spawn and drop more green circles, everyone but the jedi tanking him can stand on the stairs and not have to worry about it.
Once the Wordbearer is down, Exar and a Massassi Enforcer Blade will spawn, suprisingly enough, Exar is the easiest boss in the instance. Jedi tank Exar, everyone else kill the blade, and proceed to Exar, when he's down, take your loot and pat yourself on the back, you just beat the hardest instance in the game.
Hope this helps some people!
