Problem Area
Check for aggro ... Checking to see if we have a live target ... Yep we got a live one here houston! ... We have aggro! ...
Question: Who does it have targetted when it starts combat?
1. Nobody? 2. Yourself? 3. Yourself briefly RIGHT before combat?

Fixed a serious bug, Download most recent version please...
Running with Tweakmode off it crashes when combat ended, as I forgot to put parentheses around one of the log writes = BANG!
It'll work fine in combat mode with Log writes ON.

Ok Deceptic having a look, looks odd :/
Edit- OK with this new system I can see the problem on your end....
Look at your post and answer some questions, ive edited it.
I have a funny feeling you are targetted still when its doing all this weird combat with no mobs selected....
Also if you are a guardian? You should have it set for Priest mode, it should be mode (1) for Guardian (this isnt the problem tho)...
Edited AGAIN -
Go into your EQ2-> Controls -> Targetting Keys -> and TELL ME what 'DESELECT' IS SET AT!!!!
This could potentially be something I have overlooked... this one setting!

I'm trying this on my wiz.
deselect is set to the escape key.
it selects me, buffs, deselects me, does /decline, then goes straight into combat mode.

I pasted those settings in but my existing settings were exactly the same :\
Could you pm me you eq2 settings file? just rm out user data....

Weird !@#$%^&*! :/
Bascially the reason its LOOPING in combat, is that it has found that:
isTargetLegal=0 IS 0, which means something IS TARGETTED... I have absolutely no idea, considering you say NOTHING is targetted during this period... hmmm... 0 = PURE BLACK, therefore something has been detected in target window.
Your setup is pefect if it read that pixel as 0 - you do not need mine, its the same.
I wonder....
Edited - I have only one aswer to this and thats the DELAY time between having you targetted and it Escaping you... is that its reading you as the target before it loses you as target.... I have a fast pc, maybe timing is an issue here on slower pc's - it my last solution.
So try this new updated version that has a long delay...

Anything look out of place? For some reason, it SOMETIMES locks on a tgt, but then it stops attacking. It goes thru all of my hotbar keys (1,2,3,4...) wierd.

sometimes it sees a target, it attacks, puslls with dd, then turns off attack and casts dd over and over, then when the mob dies, it still casts HO and dd for like 5 mins with no tgt like it cant tell there is no tgt, but it isnt attacking at this point, just dd'ing.

now its tgt'ing stuff, conning it, and then turning and tgt'ing something else, conning it, and never attacking though the things its tgt'ing are blues and whites
